Box 4444, Janesville, WI, 53547-4444 ***************************************************************************** How I obtained a copy of my Summary/Breakdown letter and how to request an Intent to file. You can call the VA at 800-827-1000 and inform them that: “I would like a summary letter/disability breakdown letter to include, all my service-connected conditions, percentages and diagnostic codes.” (if you are not service-connected, you will not have one). Also, you can have them email or fax (if you don’t have a fax at home, you can download a fax app in the play store on your phone) this information, if the person doesn’t want to email or fax it, hang up and call back and speak to someone else. “Also, I would like to submit an intent to file for compensation.” ***************************************************************************** DISCLAIMER: I am not an adviser or VA representative.
